PARENTAL CONSENT & AGREEMENT FOR:

Photographs, Video, Social Media, Terms & Conditions, Member Codes and Privacy Notice.I agree to myself and / or my child’s name, photograph and video footage being shown on the Quantum Kenpo Systems website, Facebook Page, Instagram, YouTube, Twitter and other suitable media and any marketing material for promotion our service. I agree to allow sponsors of Quantum Kenpo Systems to use the above for promotion of their own service for the duration of their sponsorship agreement.

TERMS & CONDITIONS
There is no reason why I/my child should not participate in this sport. I have read, understood and accepted the rule and conditions of the Quantum Kenpo Systems, I hereby declare confirmation of those rules and fully accept the risks associated with this activity. I accept that during the hours of training I/he/she must conform to the rules and the instructor is in total charge and control.I declare that I will not do anything that will jeopardise my own or anyone else’s safety.I understand that failure to comply with any of the above could result in termination of my membership I agree my son/daughter/child named above to be physically fit and capable to participate and agree to notify Quantum Kenpo Systems of any changes to the medical information. I consent to my son/daughter/child receiving basic First Aid Treatment by instructors and to provide medication as instructed by any emergency dental, medical or Surgical treatment, including anaesthetic, as considered necessary by the medical authorities present. To the best of my knowledge, all information contained on this sheet is correct.(if under 18 please have parent or legal guardian sign)

PHYSICAL CONTACT
1. Unnecessary physical contact with young people should be avoided.
2. Typically, the club instructor may need to fix the head, hand and foot position of a student to assist in their correct position of a technique.
3. Where any form of manual/physical support is required it should be provided openly and with the consent of the young person. Physical contact can be appropriate so long as it is neither intrusive nor disturbing and the young person’s consent has been given.
